Dr. Burgujeres Bey, submitted to his Highness, the Viceroy of Egypt, a report in which he dealt with the flourishing of the medical sciences in Egypt and the existence, amongst the Egyptians, of illustrious professors and capable authors. He brought to his notice that it was high time for the directorship of the medical school to be entrusted to one of the reputed Egyptian doctors whom he believed were capable of bearing all responsibilities with no little confidence, capacity and ease. Ismail Pasha responded to his request by appointing the late Dr. M.A. El-Bakli Pasha, the surgeon who had a widespread fame for his capacity and skill. He held this office between 1863 and 1870, during which the school progressed rapidly, and thus became a stronghold for the students of the medical sciences, not only the Egyptian, but the Syrians and Armenians as well .

Dr. Rossi, the bearer of various titles, and member of many scientific institutes, alluded, in his book: «Geographia Medica dell Egitto» which he presented to the King of Italy in 1869, to the rigorous medical system followed in Egypt, and praised the scientific methods adopted in the study of medicine by saying: «These authorities with whom Egypt is rich, thanks to the care and regard of its government, constitute now a medical body, which is worth)', from all standpoints of respect and honor, and which, no doubt, equals any set of medical men in any city in Europe»..

El-Bakli Pasha resigned due to a personal a dispute with Aly Pasha Mubarak, the Minister of Education. He issued the first Arabic Medical Magazine. When the war broke out between Egypt and Abyssinia, during the reign of Ismail Pasha, he volunteered and went with Prince Hassan Pasha, as the head of doctors. He kept looking after soldiers. All Egypt appreciated him greatly. He died as a martyr, and was buried in Abyssinia.

In 1879, Mohammed Aly El-Bakli retired, and was succeeded by Gaillardot Bey from 1879 to 1882. Following the retirement on pension of Gaillardot Bey, the directorship of Kasr El-Aini School was, in 1883, accorded to Prof. Eisa Hamid Pasha because of his influence and the regard bestowed on him by the authorities. The school was rebuilt to accommodate 500 students. He utilized his visit to the European capitals and medical schools to formulate a curriculum, which he submitted to the Ministry of Education. With brisk paces, the School of Medicine reset its progress in full swing : so much so that Virchow, a German authority, when visiting it in 1883, showered praises on its Director, Eisa Pasha, and admired its modern organization.

In 1885, the doors of the out-patient department were opened to the public who thronged to it by the thousands and got Lord Cromer to say in his report: «I rather apprehend that it will have to be closed owing to the increase in the number of those who attend it». This splendid organization prevailed until Eisa Pasha resigned in October 1889.

The professors founded a Clinical Medical Association and issued a monthly review «Maggalatel-Sehha» in Arabic in which they reviewed published scientific artiest and attempted to redress the current medical problems.

Kasr El-Aini remained in link with the school until 1884, the time when Hassan Mahmoud Pasha became Director of the Public health Departments. He then proposed that the two should be disconnected. In 1890. Hassan Pasha Mahmoud was appointed as Director of the school, and Mr. Herbert Milton as Surgeon and Resident Director of the hospital..
